4. Choose "Terms of Payment", ex. click "Annually"

1. Complete Client Application

2. Click "NEXT"

3. Complete info needed, scroll down then click "NEXT"

4. Enter Beneficiaries then click "NEXT"

5. Complete medical history questionnaire then click "NEXT" (for fast approval of the policy put "NO" to all questions)

6. Check the "Authorization" box then Submit

7. Sign and submit signature

PAYMENT

1. Make a payment, click "Bank Deposit" or "Online Bank Transfer"

2. Checkout and click "Next Step"

3. Choose your preferred bank

4. Deposit/Transfer your payment

5. You will receive an email and click "CLICK HERE TO SEND US PROOF OF PAYMENT"

2. Upload deposit slip

3. Success! You already started your savings program!

4. You will receive a separate email from Kaiser in 10-15 days for your members code and online account once your Policy is approved
